These non-responsive practitioners must provide OED a completed Data Sheet by March 5, 2004. Those failing to reply will be removed from the register. However, upon furnishing the required information and payment of the Petition Fee at 37 CFR § 1.21(a)(3), a late individual so removed may be reinstated.
